RBCs undergo a breakdown process after their normal life cycle and yield heme, which is further broken down into unconjugated bilirubin.
Unconjugated bilirubin is a lipid-soluble molecule that is insoluble in aqueous solutions and is therefore bound to albumin for transport to the liver, where it is ultimately conjugated by UDP-glucuronyl-transferase.
Conditions that cause an unconjugated hyperbilirubinemia are either due to an increased breakdown of RBCs or a decreased conjugation of bilirubin.
Conditions that cause a conjugated hyperbilirubinemia are either due to a deficiency of a canalicular membrane transporter or a blockage of the bile ducts.
Unconjugated hyperbilirubinemia will cause jaundice (yellow pigmentation of the skin) whereas conjugated hyperbilirubinemia will cause pale stools and dark urine.
