About My Community Festival 2021
Organised by non-profit organisation My Community - with support from the Singapore Tourism Board as part of the ongoing Passion Made Possible campaign – the festival is co-curated and co-designed with heritage practitioners, business owners, legacy brands and other cultural institutions. The festival will provide opportunities for festival-goers to meet fellow neighbours who will open up doors to the hidden gems within our neighbourhoods where people live, work and play.
About National University Healthcare System (NUHS) Community Care Team
The NUHS Community Care Team is based in our heartlands. Comprising nurses and care coordinators, the team supports the Ministry of Health’s ‘Beyond Hospital to Community’ efforts by delivering care to the community. Some of the initiatives from the Community Care Team include the mobile Community Health Posts (CHPs) and the Hospital-to-Home Programme to ensure that the care needs of all members in the region are always met.
