A 30 weeks-twin pregnancy antenatal mother was referred to Kailash hospital & Neuro Institute, Sector 71, Noida in February 2022. Mother`s ultrasound report showed monochorionic (shared common placenta) discordant twins (weight difference of more than 20%) with decreased blood flow to the smaller baby. She also had uncontrolled high blood pressure and hence emergency Cesarean was done to deliver the babies. First twin weighed 1.5 kg and the 2nd twin weighed 1 kg at birth. The larger twin`s condition was critical requiring ventilator along with surfactant (medicine given into the lungs for lung development). He was kept on ventilator for 3 days followed by 3 days of oxygen support. The smaller twin also sick, had breathing difficulty for which he was kept on CPAP for 2 days. Feeds were given very carefully and slowly due to the risk of Necrotising enterocolitis. The babies were treated at the hospital's level III Neonatal Intensive Care Unit (NICU) by the Neonatology team led by Dr Ankit Soni. The 1st twin was discharged after 24 days at 1.8 kg weight and the smaller twin was discharged after 35 days of NICU stay with a weight of 1.5 kg without any complications.
