In this video, Dr. Jay Mehta is going to explain about Micro-TESE stands for Microsurgical Testicular Sperm Extraction, treatment for Non-Obstructive Azoospermia .
Azoospermia is a common cause of male infertility. It is characterized by Zero sperm count or Nil Sperm Count. In Azoospermia, the more common variant is Non- Obstructive Azoospermia, which typically means that testes are structurally normal but there is an inherent defect in sperm production occurring inside the testes.
In Non- Obstructive Azoospermia, the patient is more likely to have a smaller testis size, slightly abnormally located, and slightly softer inconsistency. There could be variable hormonal reports predominantly with elevated levels of FSH, mildly elevated levels of Estradiol, and low to the normal level of testosterone. In Non- Obstructive Azoospermia, the treatment of choice for a male patient to have a child of his own is to perform Micro-Dissection.
How Micro-Dissection TESE is performed?
In Micro-Dissection, the testis is opened up and a microscope is mounted on the testis for magnification. This is done because Inside the testis there are several compartments and these have seminiferous tubules in them which cater to sperm production.
So inside the testes, there could be some areas that might be producing healthy sperms that are retrieved with the help of a Microdissection production which is helpful for ICSI.
Risks Involved in Micro-TESE
The procedure takes about 2 to 2 ½ hours. This procedure is done under spinal or general anesthesia. This procedure requires the highest level of concentration, patience, and clinical experience Patients undergoing Micro-TESE are relieved within 24 hours. Patients are allowed to go back to work after 48 hours of surgery.
There could be slight post-operative pain for a few hours. Mild painkillers and a 3-5day antibiotic course can be given to the patients.
In Non-obstructive azoospermia, based on the hormonal parameters the doctors can give pre-medications in the form of tablets or injections which can last up to3- 6 months if deemed necessary.
The sperm retrieval rate using Micro-TESE is about 55-70%. Micro-TESE is one of the best hope for patients with Non-Obstructive Azoospermia to have a child with his own sperms.
If during the procedure, if the doctors find the excess of sperms, these sperms can now be preserved using Sperm Vitrification Process
