Many patients with a chronic illness, such as acute kidney injury, find it difficult to understand health information, which can lead to poor health outcomes, reduced quality of life, and increased costs to the NHS.

Royal Berkshire NHS Foundation Trust wanted to redesign the delivery of their outpatient service, with greater focus on virtual care, and remote ways of providing accessible and impactful patient education.

The Trust looked for a solution to support patients with chronic kidney disease and acute kidney injury, by presenting engaging and accessible health information to help them understand their diagnosis, staging, and how to look after their renal health.

The Trust worked with Cognitant to develop an engaging and accessible health information programme, using avatars, to help patients understand their diagnosis, staging and how to look after their renal health.

The resource is designed to be used any time, anywhere, allowing patients to view the information in the comfort of their own home or their place of choice. The resource can be saved and returned to at any time as well as shared with friends, families and carers.

To improve the accessibility of the resource, it has been translated and culturally adapted into Polish, Nepali, Urdu and Punjab.
