After Bendy kills the Projectionist on Chapter 4 and you open the door to the boss fight with Boris, double back to the ink room where the Projectionist was, and up the stairs your find the selector wheel for the Thick Ink Machine. This wheel has the plunger.

When you come to fight Boris, add the wheel to the Machine and select the plunger. Hit Boris 3 times with the plunger and the Trophy/Achievement is yours!
