This was a little challenging as the lipoma was just off to the left of her upper back tattoo. If I made the incision where I would have liked (right over the lipoma), it would have destroyed her tattoo and she would have a scar plus a deformed tattoo.

So I made my incision where I thought I could hide the scar as much as possible, which was a relatively mild curvilinear part of the tattoo.

I excised the lipoma completely (don’t worry, all the pieces were removed) and sewed it back up in multiple layers. I did a 2-0 vicryl for the deep layers. For the skin I used 3-0 vicryl for the deep dermal layer and then 4-0 monocryl for the subcuticular layer. Then I put glue on top.

The last photos are from her two week post op appointment. There is a little deformity to the tattoo line but we were both pretty happy with the result. Once the glue comes off completely, she was advised to apply scar creme and to wear sunscreen if she goes outside (UV light on a healing incision can cause darkening of the scar).
