Throughout Resident Evil 4, Ashley Graham relies on Leon S. Kennedy for protection as she is unable to defend herself against enemies and traps. As a result, players must instruct Ashley to either "wait" in her current location while Leon progresses or "hide" in nearby crates until Leon signals the all-clear with a whistle. When in danger, Ashley will repeatedly call out "Leon, help!" to alert the player. Failure to rescue her will result in either her kidnapping or death, ending the game instantly.

While most enemies will simply damage Ashley as they would with Leon, some, such as El Gigante, will kill her if she is not rescued in time. Regular Ganados and Cultists will attempt to kidnap Ashley, and players must save her before she is taken outside of the area. It is best to aim for the lower body of the Ganado with bullets or the knife to avoid accidentally killing Ashley.

Unlike Leon, Ashley cannot free herself from bear traps, so players must manually free her. Additionally, her health will gradually decrease to as low as zero until she is rescued, but she will not die. If Ashley becomes trapped, players will hear her struggling against the bear trap while occasionally pleading for help. In this state, she is vulnerable to being hit by Leon or being carried away by a Ganado.

Ashley will not jump down from ladders, so players must catch her using the "Catch" command prompt. However, there are some ledges from which she can safely jump down.

At certain points in the game, Leon can have Ashley operate cranks in areas that are inaccessible to him, such as the water room in the castle. In some instances, players can choose to operate the cranks themselves or ask Ashley to do it instead, such as with the wrecking ball.
