65 is the big birthday. You’ll be eligible for Medicare if you are over 65 and a U.S. citizen OR you are over 65 and a permanent legal resident who has lived in the United States for at least five years.
It comes as a surprise to many people that there are premiums associated with Medicare. However, many people are eligible for premium free Medicare Part A (which is your hospital insurance).
So will qualify for premium free Part A if:
You are receiving social security or railroad retirement benefits OR have worked long enough (40 quarters) to be eligible for benefits even if you are not collecting them yet
· You are also eligible if you are over 65 and you or your spouse is a government employee or retiree who has NOT paid into social security, but who HAS paid into Medicare payroll taxes while working[S4] , meaning a Medicare-covered job.
If you are ineligible for premium free Part A, it can be quite expensive.
Many people are automatically enrolled. If this is the case, you will receive your medicare card in the mail around your 65th birthday. Your initial enrollment period is the 7 months surrounding your 65th birthday: 3 months before, the month of, and three months after. If you do not receive your card during this time, make sure to contact social security.
Under 65
You may also qualify for Medicare if you are under 65, but only under specific circumstances.
For example if you receive social security or railroad retirement benefits for a disability, then you would be eligible for Medicare after 24 months.
If you have ALS or ESRD, you would be eligible much more quickly.
Remember, there will always be a premium for Medicare Part B and that is based on your income. The minimum monthly Medicare Part B premium in 2023 is $164.90 per person, and they only go higher, depending on your income.
However there are medicare savings programs which may be available to you based on your income and assets. These can help you to pay for Part A and Part B and can even offer Part D savings- prescription drug plans.
Enroll/Defer
Another thing you absolutely must do is choose to enroll or possibly defer. There are different enrollment periods and options.
You will likely be automatically enrolled in Original Medicare (Part A & Part B) when you are first eligible. This is the 7 month period surrounding your 65th birthday: 3 months before, the month of: and 3 months after. If you are not automatically enrolled during this time and you don’t receive your card in the mail, make sure to contact social security.
Now, you may choose to defer Medicare, specifically Part B. There is a premium for Medicare Part B, so if you are receiving creditable coverage (perhaps through an employer if you are still working) you may want to defer Medicare Part B. Afterall, there is no need to pay extra if you are already receiving coverage.
If you choose to defer Part B, follow the instructions on the back of the card and send it back. If you keep the card, then you keep Part B and you will be charged for it.
If you are interested in Part C or Part D, you can enroll when you are first eligible for Medicare or during the AEP.
Be warned, there are late penalties for Part B and Part D so make sure that you maintain creditable coverage.
