This is the most difficult Chalice Dungeon I've found in Bloodborne, and this video shows how to beat it, or at least that it can be beat. The dungeon glyph/code is 74ezm9bw, but you are required to place a fetid, rotted, cursed, root Isz Chalice before you're even allowed to enter.
