This patient was treated with Tranexamic Acid, a new pill, for her melasma. This pill is a great choice for patients who have already tried and failed to achieve satisfactory control (or sustained clearing) of their melasma through a combination of topicals, lasers, and sun protection. Most patients will find they react well to a combination approach, but for those that are still struggling with melasma, tranexamic acid is a safe and effective treatment.
Tranexamic acid(TXA) pills have been shown to be effective in clinical studies, at a much lower dose than is typically taken for other symptoms, like blood clotting issues. The oral (pill) form is far more potent than topicals, and has been shown to be effective for a variety of patients. However, this does require management by a physician, and a prescription for the medication.

Melasma is a brown discoloration of the skin that's usually triggered by hormonal changes. It's sometimes referred to as "pregnancy mask" and it can be very frustrating to deal with. Sun exposure does make melasma worse, and hormonal fluctuations can also make melasma worse.
