Where the patient has neglected the shoulder problems due to injury or trauma or because they will get arthritis when arthritis sets in it is not a good thing to happen because the surgery that is there is not of a good longevity. If it happens after a certain age, says 60’s and 70’s, then we have good options called as replacement. One is we do a Total Shoulder Replacement, that is both the cup and the bone of the shoulder is replaced according to the same anatomical position and the cup will come to the cup position and the ball will come to the ball position So that is total anatomical position. Sometimes what happens is the rotator cuff muscle will be completely tom, you will not be able to repair but because it is very tight and it is not able to come to its position to repair. So this type of people, they will develop arthritis. So here the rotator cuff muscle that is absent, here we cannot go for conventional kind of a Shoulder Replacement. We have to go for a reverse Shoulder Replacement. So what is Reverse Shoulder Replacement? Here instead of the cup, the ball has to be there and the ball will become a cup here are where the cup is here we have to replace the ball. So it is non anatomical position and the rotator cuff muscle that is helping, will not be there. So here the deltoid muscle or the shoulder muscle, should be very good and very active which can help in movement of the shoulder. So the function of the shoulder if it is very active, then we can see the replacement called as reverse Total Shoulder Replacement, and it is done when the rotator cuff muscle is absent or in a condition where it is totally absent or not efficient and not repairable. So this is the type of retaken what we do. So what happens is in patients who have multiple fractures of the ball head an whatever fracture fixation cannot be fixed properly so we have to take out all the pieces and replace only the bone. So here we call it as hemiarthroplasty. So there are different types of surgeries in the shoulder. So it all depends on the problems. So hemi replacement is partial and from total we have to go for the reverse shoulder arthroplasty.
