Surgical video showing treatment of an iris coloboma and cataract causing photophobia using femtosecond laser cataract surgery with implantation of an artificial iris aniridia segment (Morcher 96G) and toric IOL with Verion digital image guidance system.
A 35 year old man complained of reduced vision and chronic photophobia due to
- cataract
- congenital iris and chorioretinal coloboma
- amblyopia
- torsional nystagmus
- astigmatism
Pre-op visual acuity = 6/36 (20/120)
Surgery consisted of femtosecond laser cataract surgery with implantation of a toric IOL assisted by a Verion digital image guidance system to improve visual acuity and treat corneal astigmatism.
Additionally, a Morcher 96G artificial iris segment was implanted in the capsular bag to block light entry through the iris coloboma to treat the chronic photophobia. A capsular tension ring was also implanted to prevent long-term capsular bag contraction which can lead to displacement of the iris segment towards the visual axis or away from the coloboma.
At 4 days post-op:
Unaided visual acuity = 6/18 (20/60) (limited by amblyopia)
Symptoms of photophobia had resolved completely
