Play until you have enough skill points to upgrade your focus skills within the Skill Tree menu. Then you're going to want to play the campaign, have the cops called on you, or preferably go to a gang hideout. Your goal is to have at least 5-8 enemies on your screen at any point while having a good rated assault rifle. Pop out, activate focus, and kill 4+ enemies in one instance of focus to gain the achievement/trophy.
