Hazel Johnson from Stewartsville, Va., was diagnosed with heart failure in 2014. In order to manage her symptoms proactively, the Heart Failure Team at Carilion Clinic impanted the CardioMEMs device into Hazel's pulmonary artery. This device can remotely monitor Hazel's pulmonary levels and alert her heart failure team of an issue well before symptoms arise. This device is helping reduce hospital re-admissions for heart failure patients.
