Hernia in children is always congenital, because of developmental defect. Indirect inguinal hernia can be congenital but the defect will manifest later in adults, whereas direct inguinal hernia is always acquired. What is the difference between these two? So in hernia in children, the internal ring and external ring are together because there is no development as yet and during the development in boys, the sac also comes along with it and in adult acquired indirect inguinal hernia, there may be a persistent sac since a very long time and suddenly they start manifesting. So it can be acquired or it can be congenial. So whereas direct inguinal hernia it can be in the posterior wall of the inguinal area. So it is a weakness of the abdominal wall that causes Direct inguinal hernia, it is always acquired.
