This video is a demonstration of the Alar Ligament Test. The purpose of the Alar Ligament Test is to test the integrity of the Alar Ligament and thus upper cervical stability where a positive test could indicate a potential diagnosis for an Alar Ligament Injury.
With your patient lying supine, place one hand on the occiput and the other palpating the spinous process of C2. Then, laterally flex your patient's head to one side where you should feel the spinous process to the opposite side; repeat it on the other side.
A positive finding would be an absence of the spinous process moving to the opposite side, which could be indicative of a potential diagnosis of an Alar Ligament Injury.
