Patients coming in for follicle scans to see how their ovaries are responding to stimulation often ask me “why are you measuring my lining?!”.
We measure the endometrial lining whenever we are monitoring follicle growth because the thickness and pattern of the endometrium (the inner lining of the uterus) can significantly impact the likelihood of successful embryo implantation… but even if there is no plan for embryo transfer (egg/embryo freezing for fertility preservation), the lining thickness is a bio marker that indicates rising estrogen levels (coming from maturing follicles)….and knowing how an individual’s uterine lining behaves when stimulated can be helpful information for the future when embryo transfer may take place.
The endometrium goes through a cycle of growth and shedding in response to hormonal changes. At the start of the menstrual cycle, the endometrium is thin. It thickens as the cycle progresses, reaching its maximum thickness around the time of ovulation. This growth is stimulated by estrogen, which is produced in increasing amounts as follicles develop in the ovaries.
After ovulation, if an embryo is present, it needs to implant into the endometrial lining. A thick and triple-layer pattern endometrium, usually seen at the time of ovulation, is considered most receptive for implantation. If the lining is too thin, it may not be able to nourish and support an embryo.
In fertility treatments like IVF (In Vitro Fertilization) or IUI (Intrauterine Insemination), the endometrial lining is closely monitored through ultrasound to ensure it is at an optimal thickness and pattern. This is one way doctors increase the likelihood of a successful pregnancy.
Monitoring both the follicle growth (which can impact estrogen production) and endometrial thickness helps doctors to provide a more comprehensive assessment and personalized treatment plan.
