Weiss Ring also known as the Martegiani Ring, is a circular peripapillary attachment that is visible within the vitreous after it detaches from the optic nerve head. When observed during cataract surgery in a patient who earlier did not complain about floaters due to a mature or dense cataract, should alert the surgeon about dealing with an unhappy patient post-op. Especially if a pre-op B-scan USG is not performed or missed, it may be a useful practice to focus on the vitreous and observe for any signs of PVD. A detailed post-operative retinal examination is warranted to look for any associated retinal tears/ detachment. Patients need to be counselled appropriately, as the floaters can be often annoying and may require YAG vitreolysis in extreme cases.
