The first and most important step in gynecomastia surgery planning is whether skin removal is needed or not. Undoubtedly, the most widely misunderstood aspect of gynecomastia is the most basic – what IS gynecomastia? What is the definition of gynecomastia? Does gynecomastia give your body a feeling? If I can feel a bump under my skin, that is gyno, right? I have a “gland”, that’s gyno, correct? SIMPLY PUT, gynecomastia is the appearance of a female-like breast in a male – so if it looks like a female breast (“manboob”) then it IS gyno. No matter if you can feel a bump under the skin, if you feel a tingling in your breast, if there’s a “gland” or not, if it looks like a female breast and you’re a male, then it is gynecomastia.

The decision of whether skin removal is the most important step of gynecomastia surgery planning is because it is irreversible, produces large chest-wide scars, has a much longer recovery period and requires a higher degree of expertise to perform correctly. When Dr. C evaluates whether a patient will need skin removal, he is determining whether the patient’s skin elasticity, the ability of the skin to retract and tighten naturally, is good enough to not require skin excision.
With many years of experience to borrow from, Dr. C will quickly know for 99% of the patients he evaluates if they will need skin removal gyno surgery. On occasion, skin removal will be performed on a patient who is wanting tighter chest skin and is willing to accept additional scarring. For those who are on the fence between non-skin removal and skin removal, Dr. C will opt for performing non-skin removal gyno surgery first and, if needed, performing skin removal surgery later in a second procedure. At the Austin Gynecomastia Center, roughly 5% of patients have skin removal performed, while roughly 95% do not need skin removal and have standard gyno removal surgery only.

All too often, surgeons are eager to declare skin removal surgery is needed with a patient’s gynecomastia surgery. Unfortunately, a reality at the Austin Gynecomastia Center is that we hear from men who received gynecomastia treatment from a surgeon who hastily declared skin removal was necessary when it truly wasn’t. Remember, skin removal can ALWAYS be performed later as a secondary procedure after the skin has been given a chance to retract naturally. Please do your research when considering which surgeon will perform your gyno surgery and compare several surgeon’s opinions to get a good idea of whether you need skin removal surgery. A little more time doing research now may just help you avoid life-long chest scarring and unnecessary surgical costs.
