What are the challenges with a somewhat smaller capsulorhexis? In most of my cataract cases, I try to make a capsulorhexis that is 5 to 5.5 mm in diameter, which will hold a 6 mm optic securely in place. Also keep in mind that there will be capsular contraction and the capsulorhexis diameter may shrink by 0.5 mm or more in the healing period.
With a smaller capsulorhexis, there are additional challenges during surgery such as removal of hemi-nuclear pieces, aspiration of the subincisional cortex, and insertion / orientation of the IOL.
