The first type, germline testing, looks at inherited gene mutations that can predispose a patient to getting cancer in the first place, Antonarakis says. These mutations can also help guide therapeutic decisions. The second type of testing is somatic testing, looking for mutations in the tumor specimen itself.

According to new updates to the NCCN guidelines, germline testing is being recommended for every patient with metastatic prostate cancer and every patient with high-risk localized nonmetastatic prostate cancer. Antonarakis says the only patients not recommended for germline testing are men who have very low-risk disease.

In addition, the guidelines advice that any patient who has lymph node metastases or distant metastases should undergo tumor somatic testing, Antonarakis says.
