Hi! I'm Dr. Michael Rivers, director of EMA ophthalmology at Modernizing Medicine. Today I'm going to demonstrate how you can document a workup in our EHR system, EMA. Here you can see a view of our chart with our histories on the left. You can go through and scroll up and down on the right-hand side which show the visits in chronological order. When I go into the exam itself, the sticky note at the top in yellow is like a post-it on the outside of a paper chart. And down at the bottom everything will fill in automatically.
I'm going to go right into my intake history and add a chief complaint. I'm going to add blurred vision and document their blurry in both the right and the left eye. I'm going to answer questions about their blurred vision. They're blurry at all distances, it's relieved by closing an eye, it's moderate in complexity, it's been going on for six months and they have a history of AMD, cataract and dry eye. They use artificial tears and they're here for blurred vision. When I save my complaint, I have a grammatically correct paragraph stating why my patient is here. I have the option to override, edit or dictate anything. And you'll notice HPI components down below are all filled in. I'm going to dictate something just so you can see how that works. The patient states they have a history of blepharitis though they are not complaining of itching today. Period.
I go to the review of systems. I'm going to mark all of the systems that I want for review today and in pretesting, I'm going to add a visual acuity. I'm going to say they are 20/20 in both eyes.
Next, I'll add pressure and will put in a pressure and copy it from the right eye to the left. And for the pupils, motility and confrontation field, I hit one button. It knocks out all three at one time and the dilating tab will note the drops that I dilated the patient with today. And I'm finished with my workup. Just one more reason that EMA should be your ophthalmology EHR system of choice.
