Otitis media with effusion is a condition, in which there is fluid in the middle ear, but no signs of acute infection.
As fluid builds up in the middle ear and Eustachian tube, it places pressure on the tympanic membrane. The pressure prevents the tympanic membrane from vibrating properly, decreases sound conduction, and therefore results in a decreased hearing.
So, hearing loss is most common and important symptom of otitis media with effusion.
There are several risk factors identified, which can cause otitis media with effusion.
For kids risk factors are: passive smoking, bottle feeding, day-care nursery, and atopy.
In many cases otitis media with effusion is associated with viral upper respiratory infections and otitis media. Otitis media with effusion is a type of otitis media itself.
Otitis media with effusion is more common among children, although it can develop at any age.
Otitis media with effusion is the most common cause of acquired hearing loss in children.
In some patients, large adenoids can also obstruct eustachian tube resulting a poorly ventilated middle ear and resulted otitis media with effusion.
Otitis media with effusion is non infective in nature. And fluid in the middle ear space are non infected.
Diagnostic:
Age appropriate audiometry and tympanometry should be tested in patients with otitis media with effusion. A ‘flat’ tympanogram will support a diagnosis of otitis media with effusion.
Treatment / Management
Otitis media with effusion generally resolves spontaneously with watchful waiting. However, if it is persistent, myringotomy with tympanostomy tube insertion is considered an effective treatment.
Decongestants, glucocorticoids, and topical antibiotics are generally not effective as treatment for non-infectious, or serous, causes of mastoid effusion. Moreover, it is recommended against using antihistamines and decongestants in children with OME.
