Your medical record in your Long Term Disability or Social Security Disability case is everything.
It's important to open up to your doctor about all of your problems and what your life is like day-to-day. That way, your medical record will accurately reflect your limitations which makes it easier to gain approval for disability insurance.

I’m in pain. I’m okay. I’m fine. Those three sentences are the last things I ever want to see in the medical record. Your medical record in your case is everything. When you go in your doctor, and trust me, I know it’s hard, it’s hard to open up, but if you’re going to be involved in a case or you’re going to have a claim for long term disability, you need to do everything you can when talking to your doctor. And the best part about it, is it’s only going to help your doctor provide better care for you. If you go in there and say, “I’m okay”, “I’m fine”, “I’m in pain”, that is not going to accurately portray the disabling limitations you have. So think about it this way- I want you, I CHALLENGE you, to go into your doctor and pretend you’re talking to the insurance company adjudicator. You know who I’m talking about, that person that you’ve been on the phone with, the person telling you they’re going to deny your case. If you tell the doctor all the disabling things you’ve told the insurance adjuster, the claims adjudicator, whoever it is, you’re going to have a great looking medical record. That medical record should capture the daily difficulties you have. Don’t just go in there and say you’re okay or you’re in pain, that does not help you, and it certainly doesn’t help your case. Remember- your medical records are everything. We can get opinions from your doctor, but if the medical records don’t back it up, that opinion is worthless. So I need you to work with me so we can fight the insurance company together.
