How To Measure Amniotic Fluid Index (AFI) On Ultrasound | Probe Position & Scanning Technique USG

- The uterus is divided into four quadrants using the umbilicus as a reference point: Right upper quadrant (RUQ), Left upper quadrant (LUQ), Right lower quadrant (RLQ), Left lower quadrant (LLUQ)
- For each quadrant, the ultrasound transducer is placed on the maternal abdomen to locate the deepest, unobstructed pocket of amniotic fluid that is free from fetal parts or umbilical cord loops.
The vertical depth (in centimeters) of this pocket is measured.
- No horizontal measurement is taken.
If there's an obstruction (like a fetal limb) that divides a pocket, neither section of that pocket is measured. Instead, look for another pocket within that quadrant.
- Calculate the AFI: The depth measurements from each of the four quadrants are added together to calculate the total AFI. The sum of these four measurements provides the AFI in centimeters.
AFI (cm) = Q1 + Q2 + Q3 + Q4
- Normal AFI: The typical range for AFI is between 5 and 25 cm.
Oligohydramnios: AFI less than 5 cm.
Polyhydramnios (or hydramnios): AFI greater than 25 cm.
