I am Dr Pradeep Kumar N, Consultant plastic surgeon at Ayu Health hospitals. Today I am here to speak about Gynecomastia. Gynecomastia is the most common condition which is prevalent in about 30% of male population. We see most of them are uncomfortable with their breast and most of the time they will try to hide with tight shirt and tight uniforms, this is quite common because during the estrogen's during the puberty there is an estrogen's shift that's happening. Most of the times the estragon surge will decrease and the normal male testosterone will take over so that the male characteristics will start appearing. If this becomes slow during the pubertal stage, at that time some amount of breast would have developed, that we commonly call it as male boobs and quiet people who are uncomfortable with this seek out and come up with options to reduce this. If see at present, when they come for surgical or when they come for consultations almost all their hormones level will be normal. There is no need for any hormone replacement. There is no need for any kind of medications for it to come back to normal and usually hormones and medicines at this stage will not help because the breast gland is already been developed to particular extent. There is one more variety where if you are very fat those fatty deposits also can accumulate in the chest causing a gynecomastia kind of thing. There is, as I already discussed, there is not much role of medicines or hormones in the treatment of this kind of gynecomastia and few people I see start to work out more and start to do more exercises to reduce the chest fat. If it is fatty there is a chance that it will reduce but if it is glandular it will not come down. The more you exercise it will start becoming more prominent because the rest of the area of fat will be removed but only the chest fat will remain. So, what to do now? There are surgical options which will help these kinds of patients. One is either you go and remove all the gland with the little slighter bigger incision. Second is you remove fat through liposuction and gland through a small port or you can just do liposuction. Don't worry about the gland. As we see in these three procedures, the most beneficial procedure is liposuction with gland removal. I usually perform two port incision gland Gynecomastia surgery where I put one port in the under arm and second port below the areola region and these two ports will achieve almost 100 % liposuction of the fatty tissue. Whatever glandular ptosis which are present which will cause bulge under nipple can be removed through the small hole which has been put underneath the areola. So, this will give a very good chest, flat chest and you will be out of the taboo of having a female boob like structure and this is a quite common surgery which is done in the day care morning. You can come get surgery done and evening you can go home, only requisite will be to use a tight compression garment for a couple of weeks so that whatever extra skin is there will shrink nicely and there will be no need to collect unnecessary fluid. This surgery will give a very perfect chest and you will be able to regain your confidence. So, feel free to talk about gynecomastia and come out and sort for surgical options. Thank you.
