She went to the orthodontist for braces.
The orthodontist told her she needed to have teeth extracted.
This would make room for her adult teeth to come down.
The orthodontist sent a letter to the dentist.
In it, he explained specifically which teeth were to be removed.
She goes to the dentist who then proceeds to extract those teeth.
Six months later patient returns to the orthodontist for follow-up.
The adult teeth still have not come in.
She's told to return in another six months.
Six months later she again returns.
The orthodontist takes x-rays.
He can't locate two adult teeth that are supposed to be there.
