This confused me on a recent speed run of Lies of P, but the reason you can't refuse giving your heart to Gepetto at the end of the game and getting the Nameless Puppet boss fight is explained in this video. Personally, I really like the nameless puppet boss fight, so I wanted to do it again. This happens if you don't have enough humanity and you are quite literally a puppet, so you can't refuse Gepetto's ask to give your heart to Carlo.
