This is a morgagnian cataract that I encountered on a recent mission trip to Southern Africa. I'm using a suture-less manual small incision extracapsular technique to remove the lens. There was a lot of lens milk that hid a much smaller dense nucleus that could be easily removed. In my experience these have been rare but fun and rewarding cases.
