Forms CA17 and CA20
1. What is next step after filling out the CA1 or CA2 form? The next step is to take your CA16 form - "Authorization to Treat" and go to a doctor to be checked and evaluated.
2. Can you go to any medical facility or does it have to be a specific doctor? You can go to any doctor - but you cannot go to a practice that uses nurse practitioners or PA's. An evaluation by one of these will not be accepted by the Dept. of Labor.
3. Once you have been checked out by the doctor, can you return to work? You can return to work only if the doctor authorizes it and gives you a note stating that you can.
4. What is the CA20? The CA20 is the "Attending Physicians Report", which the doctor fills out once he has completed examining and evaluating your situation.
5. What is the CA17? The CA17 is the form to fill out once you have a case number. Until you have the case number you cannot fill out this form and go for additional tests, X-rays etc.
