What’s a care plan? It’s a roadmap to guide your cancer care following your diagnosis and treatment of cancer. People who have completed treatment of melanoma or are currently in treatment can develop a care plan with their health care team. A care plan can:
• Detail your diagnosis
• Outline the treatments you have had or are currently taking
• Tell you who on your health care team to contact with questions or concerns
• Explain symptoms or side effects you might experience and who to call if you need help managing them
• Outline what you should do and who you should see for follow-up care
Watch the webinar, Frankly Speaking About Cancer: Survivorship Care Planning that took place on August 30, 2016, for information on survivorship care planning. This webinar shows from a social worker and a survivor’s point of view. The presentation is followed by a question and answer period with the speakers. This webinar uses a Melanoma Survivorship Care Plan, as well as a generic Cancer Survivorship Care Plan, so it will be of special interest to melanoma survivors. Survivorship care planning is a great step for survivors and caregivers who are about to finish active treatment of their cancer.
