Cataracta Nigra (Brunescens) phacoemulsification surgery case - Extremely hard Cataracta Nigra with zonular laxity, usually a case for planned extra capsular cataract extraction due to the very challenging nucleus features, approached with phaco with the assistance of the Alcon UltraChopper. Here I show the “ASTERISK TECHNIQUE”, which I developed to approach these rock hard nuclei employing the UltraChopper, ensuing minimal zonular stress and providing efficient nucleus disassembly in 8 pieces, facilitating posterior plaque cleavage and fragments phacoemulsification even with the progressive miosis observed. This was a monocular patient done with peribulbar block anesthesia; presented only mild corneal edema at the following day with prompt visual rehabilitation of a very challenging case.
