Irene Ghobrial, MD, Dana-Farber Cancer Institute, Boston, MA, shares her thoughts on the diagnosis of monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM), and how the field needs to move forward, with regard to understanding, identifying, and treating precursor conditions. Dr Ghobrial notes the importance of establishing when MGUS develops from a normal phenotype into a malignant phenotype and which patients are most at risk of progression to overt disease.
