Why Prilosec Nexium might not work for you -(Proton pump inhibitors)

Why don't drugs like aciphex, prilosec, nexium, or protonix work for me? I get this question a lot in my medical practice. The answer is these drugs are perfect if you over produce Hydrochloric acid (HCL) However, rarely do I come across a patient who's truly over produces HCL. Most of the time their Acid reflux or Gastroesophageal reflux is caused by something else: Diet, extra weight, overeating, or LOW production of hydrochloric acid.

Yes, Acid reflux can be caused by LOW PRODUCTION of HCl, you didn't read that wrong. Taking proton pump inhibitors (PPI's) like aciphex, prilosec, nexium, or protonix is perfect if you happen to be one of those few people who over produce hydrochloric acid. But if that's not you, well, you'll be asking yourself and your doctor "Why don't these drugs work for me?"

Differences in PPI metabolism — PPIs are metabolized through the hepatic cytochrome system, with CYP2C19 having the dominant role. The activity of CYP2C19 is determined to some extent by a genetic polymorphism. Approximately 5 percent of White patients and more then 10 percent of Asian patients are homozygous for a CYP2C19 mutation (ie, slow metabolizers), potentially leading to greater suppression of gastric acidity. However, in wild type homozygotes (rapid metabolizers) the effect of PPIs on gastric acidity is diminished and may contribute to PPI failure.
