"Hi, I'm Dave Esposito. I'm a board certified vascular surgeon here at Milford Vascular Institute.
And today we're going to be part of a case called ambulatory phlebectomy.
So this is a procedure that we do on patients who have symptomatic varicose veins. Varicose veins are large, unsightly veins that occur on the surface of the skin as a result of underlying venous insufficiency.
This patient is a young male, 35 years old, healthy, who for many years has had these large veins on his legs. Over the last few years, they became very symptomatic, painful, itchy and unsightly, causing him leg pain and swelling.
He came in for evaluation, and was found to have severe underlying venous insufficiency. We treated that with radio frequency ablation of the greater saphenous vein.
So we've cut off the flow to those superficial varicosities, but the problem is they're still there and they're still symptomatic.
So when that happens, what we do is this procedure, ambulatory phlebectomy. So what that means is we're going to actually remove those varicose veins.
We make tiny micro incisions. And using a phlebectomy hook, which is this little instrument here, we basically will grab the veins, tease them out, and remove them through these small incisions.
There's no stitches. It's all done under local anesthetic. And when he's done, we'll wrap his leg up, and he'll walk out and resume his normal activities.
But those big, unsightly veins will be gone. They'll be actually physically removed, and his symptoms will be gone with them.
So that's the procedure we're going to do now. I hope you enjoy it.
A little novocaine along the root of that vein using a small, little micro needle.
Tiny little incision. That incision won't even need a stitch.
What we do is using a blade here or hook, a phlebectomy hook. We're going to go in there and kind of search for the vein and try and hook it, kind of teasing it out.
And gently, we try and get as much vein out as we can through each incision, so we can make as few incisions as possible.
Gently tugging, twisting the vein. So, that's one piece of it.
We're going to work our way down. So we kind of spaced them out so we could do the least amount of incisions possible. Get the most vein out through those little incisions.
So again, these veins don't look like much when they come out, but when they're veins they're very thin-walled so that when they're pressurized, they can blow up. So, you know, that vein, when he was standing, is about as big as my pinky finger, but it's decompressed.
And that's it. You did it. So we basically hold a little pressure. We'll wrap his leg up, and he'll get up and go home, and we'll see him in a couple of weeks. And no stitches or anything."
