The Lothian IBD registry by Gareth Jones
In 2018 we wanted to generate a list of all the people living in Edinburgh with inflammatory bowel disease.
The population of interest was Lothian - the NHS healthboard catchment area for our hospitals - population approx 900,000. We interrogated multiple datasets held locally and nationally and cross-validated against each. As of 31st August 2018 we calculated the point prevalence to be 0.78%. We observed the incidence of both Crohn's disease and ulcerative colitis to be stable over the preceding 10 years.
We diagnose approx 1 person per day with IBD in Lothian. Around 80 people with IBD die every year - the vast majority from unrelated causes.
Given this mismatch between IBD incidence and mortality we observed that the prevalence was going up. This phenomenon is known as compounding prevalence. It had recently been described by Gil Kaplan in Canadian IBD populations.
Gareth modelled the prevalence of IBD over the coming decade and noted that it would surpass 1% by 2028 and keep rising despite stable incidence rates. The population will be getting older as time passes and with increasing co-morbidity; an important observation for IBD teams.
