The patient has 2 issues after a work related MVA (05/16/2018). He has low back pain and bilateral plantar foot pain (S1 Radiculopathy) consistent with his imaging studies showing an L5-S1 annular tear. He also has neck pain and bilateral arm pain and numbness (R greater than L) without corresponding findings on MRI. EMG showed bilateral ulnar neuropathy and bilateral C6 radiculopathy. His primary concerns are his lower back and foot pain so we will prioritize treatment for his lumbar spine at this time.
