This got me 4 trophies, including the platinum!
The hardest trophies in Heavy Rain are: Perfect Crime, Nerd, and All Endings. The rest of this video's description will focus on Perfect Crime and All Endings.

Now, for the All Endings achievement, you need to unlock every Epilogue on the chapter select menu. This will require about 7 playthroughs of the game's ending.
The main disjunctions in the game's story are:
■ Who (among Ethan, Shaun, Madision, Norman, Scott, and Lauren) is alive at the game's end?
■ Did Ethan sleep with and forgive Madison?
■ Was Ethan arrested twice?
■ Who was successful (among Ethan, Madision, and Norman) in reaching the Old Warehouse?

Aim for the Perfect Crime ending first. The goal is this guide is to get the shitty endings out the way so you can finish with the happy ending.
Ending #1 - For the Perfect Crime playthrough:
■ Make sure that Hassan (store-clerk), Kramer (businessman), and Lauren all die over the course of Scott's chapters.
■ Do not fail any trial as Ethan. (This lets you get the trophy for completing all trials later.)
■ Make sure that Ethan is arrested twice. Once during the Fugitive chapter, and again in On the Loose.
■ Make sure to wipe all fingerprints when Scott visits Manfred's shop. (Don't forget about wiping the front door or the display case that Lauren might have leaned on!)
■ Reject Madison. Ethan doesn't sleep with her or doesn't forgive her.
■ Make sure that Norman and Madison die. This can happen when they reach the warehouse.
CONCLUSION: With Ethan in jail and with Norman and Madision dead, the following epilogues will unlock: Helpless, Dead Heroine, Uploaded, and Unpunished.

#2 - Hero Investigators
Replay the final chapter from your previous playthrough, and allow Norman and Madison to succeed. This unlocks: Heroine, Case Closed, Innocent, and Origami's Grave

#3 - No Love and No success
Go back to On the Loose. Reject Madison again, but this time, allow Ethan to escape arrest. Have no one succeed at learning Shaun's location; (this might require that you fail Ethan's last trial on purpose). This should unlock: Orgami Blues, Square One, and Smoking Mirror.

#4 - Some Success
Go back to The Rat chapter, and allow Ethan to deduce Shaun's location. BUT, Norman still has to fail at learning Shaun's location. Madison may succeed or fail, but she must not tell Norman about Shaun's location. As Ethan, successfully save Shaun. This unlocks: A New Start and Resignation.

#5 - Tragic Love
Skip back to On the Loose. Allow Ethan to sleep with and forgive Madison. Make sure Ethan is not arrested. Make sure that Lauren lives during Trapped. Do not allow anyone to learn Shaun's location. This will unlock: Tears in the Rain and A Mother's Revenge.

#6 - Last Moment Fuck Ups
Allow Ethan, Madison, and Norman to learn Shaun's location. If you're going for the So Close trophy, allow Norman to die. Make sure that Madison fails to reach Ethan on her motorcycle. This will unlock: Ethan's Grave.

#7 - Yay
Replay the final chapter and allow Ethan, Madison, and Norman to succeed. This unlock: A New Life.
