Hi, it's Dr. Nassif. I'm answering some YouTube questions. It's Halloween, 2014. Let me read this for us. Is it necessary to use cartilage grafting for all revision rhinoplasties? I have to tell you probably on 90% of my cases, we need to do a little tweak which involves using cartilage. It is sometimes the occasion that we have to remove cartilage. It does happen, but most of the time, yes, and it could be either septal cartilage if there's some left inside the nose ... most of the time there isn't with revisions ... ear cartilage or rib cartilage.
So, in general, the answer is, yes, we have to use cartilage because, most of the time, I'm seeing patients that have basically have collapsed. I will tell you, though, when I have to do a revision on one of my own and I've grafted already, then it's usually a removal, but it's rare about removing, but sometimes we have to remove pieces of cartilage along the ala.
