Diabetes can definitely have an impact of male fertility as the elevated blood sugar levels and the elevated cholesterol levels and diabetes mellitus can cause difficulty for the male. The increase blood sugar levels in diabetes mellitus can also result in increase in level of DNA fragmentation. Increased DNA fragmentation is a cause of great concern and it can reduce the chances of pregnancy, apart from that there is also evidence that elevated blood sugars can reduce the motility can reduce the number of normal forms, that is why this should be corrected before attempting for pregnancy either naturally or by Intrauterine insemination. Diabetes mellitus in men, be it type 1 or type 2 can be definitely be transferred to the future offspring if an ICSI is performed. This is because of epigenetic modification during ICSI procedure. Diabetes in men can increase the DNA fragmentation significantly. Increase in the DNA fragmentation reduces the pregnancy rates and also increases risk of abortions, that is the reason when we do Intrauterine insemination with elevated DNA fragmentation levels ,we consider doing the processing not only by the conventional discontinuous gradient technique, we do consider doing another processing in such cases. This is what is called as the MACS or the discontinuous gradient technique. In discontinuous gradient technique, instead of the routine centrifugation, the reactive oxygen species is less and the damage to the sperms is lesser and then they are filleted against a magnetic field and the healthy ones are filtered out, and used for intrauterine insemination. This can increase the pregnancy rates in cases of DNA fragmentation being higher than 25% or 30%, but however less than 50% to a significantly higher level.
