Each participant in Stanford Medicine’s Humanwide pilot project received digital devices for measuring key health metrics at home: a pedometer, a home scale, a blood pressure cuff, and a glucometer. Data flowed directly into their electronic health records, allowing care teams to remotely monitor the readings and take action when merited. Through data from a home blood pressure cuff, doctors made a new diagnosis for Eugene, a 42-year-old patient with a predisposition for heart disease.
